Getting Started
- First, a case manager is assigned. We then conduct an initial comprehensive assessment in the home. The assessment includes details regarding the home environment, safety issues, social and family support, medical needs, and functional ability.
- Next, a plan of care is developed to meet those needs identified during the assessment. Our goal is to produce a plan of care that works toward the greatest degree of independence, safety, and well-being of the client.
- Routinely, your case manager will monitor and evaluate the effectiveness of care and the need for the continuation of services. We provide monthly monitoring and periodic home visits to monitor the care supervision of services.
